The Moline Foundation is pleased to announce a simplified form for the 2021 Scholarships.  You only need to fill out one form for all scholarships!  If you wish to apply for a scholarship based on financial need, please download your parent’s/guardian’s 1040 from last year with the social security and bank numbers marked out. [Note:  We have given financial need scholarships to middle class students as well. Please send any uploads as PDFs.]

2021 General Scholarships

We will put you in competition for all scholarships for which you qualify.  For a full list of Scholarship, click here.

You will need:

  • General Information: Name, Address, Gender, Phone Number and Email.

  • Dependent/Independent Status: Parents/Guardian and family information.

  • Education: Current and Planned School, Expected Major, ACT/SAT score, Grade Point Average, Expected Career.

  • Activities: Honors, Extracurricular, School, Work, Volunteering, and other relevant experiences. VERY Important section to fill out.  Please put everything you are doing such as band, choir, sports, volunteer at church—and what you are doing in that activity.

  • Essay: Why are you entering your chosen field of study? [Limit to 200 words.]

  • References: Upload references from two unrelated adults [teacher, coach, counselor, boss, etc.] who know your work.  

  • Transcript: Sent to The Moline Foundation via Parchment [certified program], other official program or send by Registrar to the Foundation via regular mail.

  • Optional Financial [if wish to apply based on need]: Guardian/Parents’ 1040 tax form from previous year; black out Social Security Numbers or bank account numbers.

Financial Information is needed if you wish to be considered for the following scholarships:

  • Clement T. Hanson Scholarship: Multi-year scholarship

  • Lee McAllister Memorial Scholarship

  • Doug Reynolds Rotary Scholarship

  • Sherrard Academic Foundation Scholarship—describe need only: no 1040 needed

  • Srikanth Yerra Memorial Scholarship

School/Residency Requirements:

  • Graduating senior of Moline High School, Alleman High School, United Township High School, Rock Island High School or Sherrard High School: or

  • Black Hawk College student and a live within Moline-Coal Valley School District 40 boundaries: or 

  • Junior or senior in college in Fall of 2021 and a current resident of Rock Island County.
